  • Number the pages (e.g., “Page 2 of 5”).
  • Write or stamp the certification statement above on the first page

  • Signature of the certifier
  • Full name of the certifier
  • Occupation or profession (and professional number, if applicable)
  • Official stamp or seal (if applicable)
  • Contact phone number
  • Date of certification
