Form BEN2 – Identity Verification for the RBO

A Form BEN2 is a document issued by the Central Register of Beneficial Ownership of Companies and Industrial and Provident Societies (RBO). It is required when a beneficial owner does not have a PPSN (Personal Public Service Number).

A PPSN is issued by Ireland’s Department of Social Protection to all Irish residents. When the RBO cannot verify a person’s identity through a PPSN, they will request a completed and witnessed Form BEN2 instead.

What Information Does Form BEN2 Contain?

The form includes the beneficial owner’s:

  • Full name
  • Date of birth
  • Residential address
  • Nationality

This information must be formally verified and declared to be true by the beneficial owner in the presence of a witness.

Who Can Witness a Form BEN2?

If the form is being completed outside of Ireland, the witness must be a Notary Public.
